Verification of Residency
Parent or legal guardian identification
Student's birth certificate
Immunization records for the student
Hearing and Vision screen (Kindergarten only)
Kindergarten Oral Health Assessment (Kindergarten only)
Most recent Individual Plan (IEP) or 504 Plan (Special Education students)
